The galls started underneath, but some of the cluster punched a hole through the leaf and ended on top.

Probably Quercus velutina. Are these the ones I got in the mail yesterday, same location, collected 9/26/2014 (the date you gave above is 8/26/14)?

It seems likely that the mystery galls appearing singly on Q. rubra are another Dryocosmus species, although this superficially similar one from California is a Callirhytis:
