M. heros, here's a side by side comparison,

Note the elytrial striations in M.fasciata, and the smooth 'waxed' look of heros. Great pictures!
p.s., Bracket fungus and well-rotted logs are where I have so far found them as well (here in Arkansas), never free standing mushrooms, and one actually inside the log.
