Agromyzidae
the head chaetotaxy is wrong for Clusiidae. Clusiids have a pair of setae appr. between the antennae and ocellar triangle no the head. This is an agromyzid, perhaps genus Liriomyza

 
Cerodontha
I've checked with Stephanie Boucher, the expert on the genus, and she's confirmed that this is Cerodontha (Cerodontha) dorsalis.
