Who made these three holes in a Goldenrod Gall fly gall?

Who made these three holes in a Goldenrod Gall fly gall?
Grand Rapids, Kent County, Michigan, USA
March 18, 2017
Size: holes 2 x 3 to 3 x 5 mm
This Goldenrod gall fly gall, encountered in March, has three differently-sized holes in it. They range in size from 2 x3 mm to about 3 x 5 mm. The holes are not from the Goldenrod gall fly or its parasites because the holes are too large, not circular, and the gall fly or its wasp parasites would not have emerged yet anyway. Who made these holes? One possible clue: In one of the photos, a bit of what looks like webbing is visible in the hole. All three photos show the same gall.
