Capraita sexmaculata
Am sitting here with a couple of Cornell drawers full of determined Capraita spp. in the TAMUIC.

Many of these spp. are highly variable, but I think yours is Capraita sexmaculata which is wide spread in the eastern US and Ontario. (1)

This appears to be the first determined image on the WWW... Am moving...
