We don't have many Omethids yet so: Blatchleya gracilis (Blatchley).
The antenna of the male are strange with seg 4 and 5 elaborate like a Collop*s.
Amer. Beetles says this is known from IN and OH, so this is a slight range extension.

Sorry Jeff, I do not have any good notes on this. I was hand collecting from foliage, but no record of what plants.
