Best guess
This could be E. grandis. I'm not very familiar with Western Epeorus species, but the unusual head shape (cf. Fig. 38 of the head capsule in Webb and McCafferty's "Heptageniidae of the World, PT. II," 2008) and the way the first and last gills extend under the abdomen fit well with what little I can find about the nymphs of this species.
