Can you please tell me what causes this? I see it all the time on leaves as well as in the exposed part of a tree under the bark.

Leaf miners.
Tiny larvae that eat a path through the middle of a leaf. Info here.

 
Leaf mines
These particular mines are made by agromyzid fly larvae. Other leaf mines may be made by moth, beetle, or sawfly larvae. The trails you see under bark are made by various kinds of beetle larvae.
