Caddisfly
It looks like a microcaddisfly, and possibly the salt and pepper caddis.

 
Tom,
I showed these photos to a caddisfly researcher in Plattsburgh, NY and he is of the opinion that it is a micro moth, so I've moved them back to the ID request page to get more input from both caddisfly and moth experts.
