Egg Sac.
Actually, the big white thing is the female's "ovisac," full of eggs, if my interpretation is correct. The other thing may be a female without her waxy covering. I also see an aphid trying to hide behind the leaf; and, there appear to be two armored scale insects on the twig beneath the female cottony cushion scale.
