Tenebrionidae - Neomida bicornis - Male
100 Acre Wood, Winneshiek County, Iowa, USA

Size: 4 mm
1-Jul-10: shelf fungi Trametes elegans collected.
9-Sep-10: broke open fungi and found lots of adult beetles.
8-Oct-10: this individual collected

But:
3-Jul-10 larva on fungi, but is it this species?
5-Jul-10 larva on fungi, but is it this species?
On this page (scroll down) Cristiano Lopes-Andrade says: “The larva is certainly not a Ciidae. There are Erotylidae and Tenebrionidae larvae similar to that... but I'm not specialist in these groups.”
