I don't think any hickory has five palmate leaflets like this. All the other examples I've seen of this mine have been in Iowa (see guide page).

I agree that this host plant is a species of Buckeye (Aesculus), most likely A. glabra.

I'm confident these are agromyzid mines (there are even the characteristic feeding punctures made by adult females), but (1) lists no species feeding on Juglandaceae.

 
Japanagromyza??
I used to see these all the time when I lived near Dayton. I think I actually reared one once (I'll see if I can dig it up).
Spencer (1) says that J. viridula is known from Castanea in addition to it's usual host of Quercus, and I wonder if this isn't viridula or something similar.

 
I just reared some last week
(from mines collected in Ohio last May). I was expecting an Agromyza, given that there are several that make linear mines on woody plants, but it turned out to be a Phytomyza! I'll get the specimens to Owen Lonsdale in a couple of months and will move these images accordingly when I find out what they are. I'll be interested to hear if you find your specimen(s) though.
