Hexagenia have 3 tails. This has 2. Looks like a Anthopotamus, or also known as Potomanthus...Golden Drake.

Sorry I didn't notice your comment when you made it over ten years ago; hope it's not too late now.

However, in their winged lifestages, Hexagenia species have only 2 tails, unlike Ephemera species.

There is a third tail but it is very small. Should I still move it to Potomanthus?

 
No, don't move it.
Hi Steve,

What you are seeing in the added photo is called a vestigial tail, which is a so abbreviated that it can not normally even be seen by the human eye unless aided by magnification. In any regard, this is a female imago of genus Hexagenia, and it is already properly placed in the guide.
