Size: ~5mm
I'm assuming this is a species of Derbidae.
The photo is a little blurry, but I collected a specimen (but have yet to process it). Found on dead Fagus grandifolia in a mixed wood forest, hillside.

Otiocerus degeeri
A distinctive planthopper (even when in shadow) of the family Derbidae.
