Size: 3.6 mm
Ceratagallia? uhleri? It might be missing a small transverse vein, but I don't know whether that's important.

Exitianus exitiosus
even though you can't see the characteristic head pattern from this angle, the contrsting veins are pretty characteristic; but the size is more probably 4.5 mm or larger

 
Size
I think you're correct about the size. I used the image pixels to measure leafhopper, but now (looking at the uncropped original) I see that it's sitting at an angle. This would make my figure too small. Thanks for all your help!
