Smicridea fasciatella ?
According to this Smithsonian publication:
http://tinyurl.com/3r2m3sd
there are five species in this genus reported from Arizona.
It says in the description "three transverse bands of white hair", and the diagram shows it on page 14, figure 19.

I can't tell from the photo. It looks it has scales on wings to me.
