Probably Dicosmoecus
This is probably the larva that goes with the adult Dicosmoecus that you photographed. The October Caddis larvae tend to build a tiny little "awning" into the top front of their case, and I think I can see that here. This is similar to the hooded case of the Molannidae, but not as pronounced.

This is very nice and we have very few caddisfly larvae. Don't you think that it is time to move it out of ID request?
