This is a Derbidae, Fulgoroidea, Hemiptera. The species is Neocenchrea heidemanni Ball. The nymphs of derbids have been reported to feed on fungi. Some are found on fungi under bark, and one has been reported from litter under palm trees. Adults feed above ground, but we have very few hosts reported. Many have the curved subantennal process seen in this picture--either formed from the gena or from carinae of the pronotum. It would seem that it would enhance the function of the antennae, but I have never read anything about its function.
