Agallia modesta. It seems to show the 6 dots on the head

 
Agallia constricta?
When you say 6 dots on head, do you mean 2 on the head and 4 on the pronotum? Dr. Hamilton identified a somewhat similar example here as Agallia constricta. I looked at the original description here for modesta but was not able to follow it well enough for it to be of help. Perhaps you might take a crack at it. Thanks in advance.
