"Psylla cerasi" Patch = Psylla trimaculata var cerasi
Excellent! The additional photo helped a lot. Patch described this species as Psylla cerasi in 1912 (I've uploaded her original description here for ease of comparison). In 1944, Tuthill declared the species to actually be a color variant of P. trimaculata, a species which is typically colored quite differently:

Tuthill posited that temperature could be a factor in the coloration, as all examples of var cerasi had been found in September at northerly latitudes (much like yours). Other than color, one should note the short genital segment, the wing venation with obsolete pterostigma, and very long antennae (3x head width).

This is the first example of this color form for this species on bugguide - nicely done!

What a beauty!
Do you happen to have a shot of the side? specifically I'd like to see a lateral view of the genital segment, from which a generic ID (at least) is probably possible.
