I believe it is Octoglena anura, but another should confirm before placing at species level.

Do you know how common this species is? Also, do you know of any other species of Polyzonids in the pacific northwest?

I am not as familiar with the millipedes from your area, but from Hoffman's 1999 checklist (1) it appears most of them are found in California and Southwest Oregon.

I would not consider your location SW, but I don't know how strict the ranges are.

Oregon Range

I would bet that the range is somewhat strict. Southwestern Oregon has very similar habitat to California, which starts to fade out around Douglas County area.
