Size: Very Small
This one was small enough that I did not notice it until after I brought the image up on the computer. If I had noticed it I would have tried to get in closer and maybe get a better image. Based on the images and comments I found on BugGuide, I think that this is (Sminthurinus quadrimaculatus forma bipunctatus) but the white "eyebrows" don't quite come together. Anyhow, your expert opinions on the correctness of my determination would be greatly appreciated

Indeed the diff is the eyebrows. Both forms have been described by Maynard in 1951.
