On yarrow, Achillea millefolium. Swollen stem, wilted tip...looks like a fly or a lep to me, but I didn't tear into it to find out. Perhaps someone will recognize this sign. If not, does BG have an "Unidentified herbivory / feeding sign" page?

I would assume some sort of stem-boring lep. There are pages for mysteries like this, more or less corresponding with the chapters of my book, here.
