Looks like a three-lined potato beetle but it has a dark head... looks more like the Lema nigrovittata in bugguide with the black head. But they are western species - Arizona ...
It was on a Pumpkin plant leaf.

Thanks for the ID - we are not growing any cucumbers but this one was on a pumpkin leaf briefly - maybe it was checking it out and then left!

 
The info page lists the hosts as Cucurbitaceae
pumpkins belong to that family also so a possible host.
