From what I can see this should be a species of Vanduzea (possibly triguttata) because veins R and M are fused towards the base (http://treehoppers.insectmuseum.org/public/figure/show_zoom/7948), not separate (http://treehoppers.insectmuseum.org/public/figure/show_zoom/7946)
