Baileya levitans
(or Pale Baileya). Note the greatly reduced basal patch, the prominent tooth in the AM line, and the smoothly rounded hump in the PM line; compare photos of levitans and ophthalmica at CBIF.

This is a very nice shot to have; it's a much better photo and specimen than mine in every respect.
