Leafhopper adult (Update: ID'd as Henribautia nigricephala)
Leafhopper nymph (Update: ID'd as Typhlocybinae)
Leaf mine #1 (Update: adult emerged and ID'd as Cameraria caryaefoliella)
Leaf mine #2 (Update: ID'd as Stigmella)

Tree and branches somewhat drooping. A young pecan tree?

Update 6/18/17: I'm pretty certain it's a pecan. I confirmed that a nearby tree that looked exactly the same, only larger, was a pecan tree.
