If you can avoid using the narrow-mouthed glass vials for oak leaves (and thicker/larger leaves in general), it would eliminate the risk of the larvae being squished as the leaves are put in the vial (or removed from it), which I think may have happened here. I end up transferring everything to plastic vials anyway, because it's harder to see when an adult has emerged in a glass vial (it can hide up in the lid) and harder to get adults out without damaging or losing them.
