Valenzuela sp.
This is one of the "honey wings", Family Caeciliusidae. Conveniently this family only has two genera in the US and Canada. The genus Xanthocaecilius has two species with yellow and green eyes respectively (see photo#87070). Therefore this is individual is from the other genus Valenzuela (a great sounding name!). Sorry I can't take it farther yet.
