Size: ~1cm
I think this is a mayfly, but the eyes didn't look like any of the species on the guide. They're very red and close together when seen from a dorsal view.

The wings and the appendage on the abdomen have been damaged (I first noticed it after it had flown into a bag of dog treats I was carrying).
