Probably...
Heptageniidae. In most cases, this is a problematic angle for a mayfly ID, and the focus is not very clear. One interesting thing about this specimen is that the front and middle legs on the right side have been regenerated. This was probably due to damage that occurred in one of the nymphal instars.
