Leaf collected. Pupa still present within the mine. I can’t seem to find much information on North American leaf miners that use orchids.

The frass pattern is consistent with other scathophagid mines I've seen on orchids. This species ID is still tentative though.

Well, there aren't any known to use Goodyera!
I don't suppose you collected this? Parallelomma vittatum (Scathophagidae) mines leaves of various orchids, but the frass pattern in the linear part of this mine looks very agromyzid-like. If you checked the underside of the leaf, a Parallelomma vittatum mine would have a conspicuous white eggshell glued to the beginning, and an agromyzid would not. I suspect that's a dead larva in there rather than a pupa, but in any case I hope you can find some more and try rearing some adults!
