Size: about 12 mm
is this the only kind of checkered beetle in the West? I want to make sure I have the ID correct.

The ID and spelling is almost correct,
Trichodes ornatus - Ornate Checkered Beetle. Not the only one in the west by a long shot. While logged into BG do a search for Checkered Beetle, click on any photo, above the photo that you selected you will see a tree leading to the image you selected. In this case select 'Checkered Beetles' then click 'Data' scroll down then click 'Utah. This will bring up all the Checkered Beetles that have been found in Utah. I hope this is helpful. I use this a lot to narrow my searches for insects that I have photographed/collected.
