I'm surprised that it took four years to ID this common moth.
Notes:
1) The fore-wing's dark, discal spots are very close to the (missing) AM lines.
2) The hind-wings are very pointed.
3) The fore-wing's PM lines are reduced and (sometimes) composed of light-spots, with a slight curve. (dash near costa) Usually this line is much more prominent, which explains why this took so long to ID.
4) The hind-wing's PM lines are straight across the middle, near the apex of the abdomen.
5) The head is brightly colored, around the antennae.
6) The location is east of the Rocky Mountains.
