Neoleptophlebia memorialis

I believe this male imago to be Neoleptophlebia memorialis, but missing its 3rd tail.

I think you can sort of see a part of it's third tail at the bottom there-- the mayfly had flown itself right into a spider's web, so it's possible it pulled the tail out in the struggle to escape.
