Case-bearer caterpillar - Homaledra octagonella

Can anyone give me some information on this caterpillar? What kind of moth does it turn into? How does it make the case? Is the case made from its frass and silk? Note how perfectly the small black pieces are situated in the case. Any information would be appreciated!

Possible species octagonella? from previous photos is not in the Bug Guide taxonomic listing.
