The gall from the first two images was split open to reveal the immature insect in the third image. These galls were very common throughout a patch of grass.

The gall looks like a fungus
The gall looks like a fungus attacking the grass, similar to Ergots on wheat.

Hmm, it's possible I messed up and the small insect didn't come from the gall, but I doubt it. I remember gently snapping the gall in half and pulling the insect out. Are there insects that parasitize fungal galls?
