It matches up with the picture in National Audubon's Field Guide to Insects and Spiders, and Jim McClarin has a few images here too.

Lema trilinea
Unless someone has changed the name on me, this is Lema trilinea.

I see that when I checked the Info tab for this species, it's been changed to Lema daturaphila and the synonym is Lema trilineata.
