Comments

Does anyone have page 106 ?
Page 106 is missing.
The PDF has a duplicate of page 104 between pages 105 and 107.

I have -
a hard copy of that monograph and should be able to scan that page and email it to you. Feel free to contact me through the email address on my bio page.

This reference -
added a long time ago here:
https://bugguide.net/node/view/1539675

(I just fixed the link, good reminder)

Would suggest deleting this one so we don't have a duplicate?
