Heptagenia adaequata
Based on Webb and McCafferty 2008 figure 198 I would say Heptagenia adaequata. The tergite markings fit but most important is the distinct point on the bridge of the clypeus, but the lateroparapsidal blackish markings slightly forward-lateral to the scutellum. Heptagenia is the genus and adaequata is very likely just the scutellum is a little dark.
