Since we don't have a clear view of the adult, and don't know what the host plant is (or, for that matter, what the nymphs of all the other species look like) genus is probably the best we can do here.

Best I can do with adult. It seems to have a chance of showing resemblance perhaps to Gargaphia tiliae.

Not able to determine leaf. It would be nice if we could call it American Basswood, but the veins on each side of the midrib for this species seem to join at the same points on the midrib, whereas on this leaf they are alternating at different points on midrib.
