Phryganeidae
The striped head and the case of spiral or stacked cylinders of plant material are characteristics of many members of this family. This might be Ptilostomis. (The markings on the prothorax look like that genus.)

 
Ptilostomis
Ptilostomis is a good guess. Many giant case-makers build spiral cases, but this one is a set of stacked cylinders, which is typical of the genus Ptilostomis. I'v seen the genus on many nearby rivers, including both the Farmington and the Deerfield rivers, where the water is unpolluted.
