Tim, I'll put it in the family for now, and see if Don can go further with the specimen.

Heptageniidae
Might be Cinygmula- the gills do not seem to have fibrilliform (thread like) components and the maxillary palps (at least on the right side of the head) protude from the sides of the head.

 
Correction
I didn't notice this before but it looks like the first and last pairs of abdominal gills are expanded, which would indicate Rhithrogena.
