This is an interesting leafhopper with eye spots. Possibly
Amblysellus curtisii
or at least a relative?

Graminella villica
Per Andy Hamilton, "the head spots are rounder, the yellow band across the pronotum is on the front half, not near the scutellum, and the wings are narrower."
