Size: ~30mm
I do not know the ID of the host plant. I think it was Button Bush (Cephalanthus occidentalis) but not 100% sure. I'll have to go back and check again.

Moved

Too many pair of abdominal prolegs
to be a lepidopteran. Rather it is a sawfly larva. Here is one that we photographed on Buttonbush in northeastern Illinois

know sawfly larvae got this big! Thanks for setting me straight :^)
