Moved
Moved from Psylloidea. Presumably the same as this one:

Psyllids--if this is alder, maybe cottony alder psyllid? I haven't found an image of an adult to compare with this one.

Host Plant
I have uploaded a view of the entire leaf. From what I can tell the host plant is a specimen of Alnus incana which is noted to prefer higher elevations along watercourses and poorly drained areas.

Alnus incana
I agree this is Gray Alder (Alnus incana) in the birch family (Betulaceae).

I'll check
I see if I have any better images of the host plant for identification purposes
