Should the genus
Otiorhynchus
be placed in the subfamily Entiminae (as it is in the Guide) or in Otiorhynchinae? And what about the other genera currently placed in
Entiminae
?

Aclaration
Recently the subfamily Otiorhynchinae was placed as a tribe (Otiorhynchini) in the Subfamily Entiminae. Otiorrhynchus is more diverse in Europe.
In the book American Beetles (2002) you can find a Key with 124 genera in the neartic.
The classification of the group have a lot of problems.
