those mushrooms [=turbinate eyes] are upper partitions...
...of male compound eyes, which are differentiated in most mayfly males, but to a various degree in different families, and in Baetidae this dimorphism goes far beyond average and may seem outrageous

Nice photo. This is a male subimago of family Baetidae.

Mayfly
They're the eyes, making this a male I do believe. Maybe Eurylophella(Ephemerellidae). I don't know much about Mayflies so don't take that too seriously.

 
I think they may be a
second set of eyes. See the comment about "turbinate eyes" on this post.

I could be mistaken, though, so it'll be interesting to see what the experts have to say about this one. :)
