Is Madarellus known to make galls?
Looking in Felt (1), I would have supposed this was Ampeloglypter ater.

I defer to your expertise. Wish I had a photo of Ampeloglypter ater. (But then, maybe I do.)

but hopefully someone who knows about weevils will comment on my question/suggestion. I guess my question was partly, what led you to suspect that this is Madarellus undulatus? There seems to be some confusion with these two genera... see my comment here:
