Is Madarellus known to make galls?
Looking in Felt (1), I would have supposed this was Ampeloglypter ater.

No expertise here...
but hopefully someone who knows about weevils will comment on my question/suggestion. I guess my question was partly, what led you to suspect that this is Madarellus undulatus? There seems to be some confusion with these two genera... see my comment here:
