Size: ~7 mm
This hopper was just sitting on a rotting log. Identified to Genus Otiocerus via BugGuide images and comments. Looks closest to Oticerus wolfii to me, maybe a newly molted individual.

Thanks Dr. Hamilton. I did find a nymph about 2 feet from this individual just before I found this specimen. I have posted images to the ID request queue which I'll link below. I had meant to post it sooner, but I have a backlog of images. I do not know if it is related, but looking at the facial structure, especially the shape of the top of the head, the eyes, and the antennae there seem to be similarities.

 
Otiocerus nymph
There is one drawing known of this nymph, clearly from a dried specimen as the central membranes are shrunken so the tergites are all crowded together on the back. The detail is not good but it is clearly the same genus that you have photographed.
